According to Peter Jackson in the film’s commentary track on the DVD & Blu-Ray, the above image is the sole thing that caused the MPAA to give The Battle of Five Armies a R rating. Legolas’ small dagger going through Bolg’s hand caused a PG-13 movie to be rated R when it was added to the film in the Extended Edition.

That’s right. No additional blood, no severed limbs, nothing more than a small dagger going though a CGI creature’s hand caused the MPAA to increase the rating from a PG-13 to a R. They are completely useless…
